Community Health Needs Assessment (CHNA)
Who: Baldwin Area Medical Center, Hudson Hospital & Clinic, River Falls Area Hospital, Westfields Hospital & Clinic, Pierce County Public Health and St. Croix County Public Health lead the partnership to plan and implement an organized, two-county, community-based approach for creating and maintaining healthy communities.
What: a systematic process involving the community, to identify and analyze community health needs and assets in order to prioritize these needs, and to plan and act upon unmet community health needs (Assessing and Addressing Community Health Needs, CHA 2011)
Where: Pierce & St. Croix Counties
Why: As part of the Affordable Care Act (ACA), non-profit hospitals complete a community needs health assessment (CHNA) every three years. Hospitals must collect input from the community, adopt a plan to address community needs and then report on these activities. While not related to ACA requirements, the state of Wisconsin also requires that public health departments conduct assessments “regularly.”
