One great way to get involved is to take our Community Survey.

Three years ago, many of you gave invaluable input to help shape the priorities of the Healthier Together Coalition through our Community Health Needs Assessment and Planning process. Since then, we’ve been working hard on areas of mental health, substance use disorder, nutrition and physical activity.

It is now time to revisit our health priorities and complete another community health survey. Please help us share our survey to as many community members and organizations as possible.

Community Survey


Each of us can play an important role in community health improvement whether in our homes, schools, workplaces, churches, or in our communities. Encouraging and supporting healthy behaviors from the start is so much easier than altering unhealthy habits.

Because community health improvement is a shared responsibility among us all, everyone is invited to join the efforts of Healthier Together – Pierce & St. Croix Counties. Below are the involvement levels. If you’d like to get involved, let us know!

  • Organizational Partners: Organizations committed to partnering with the coalition, ideally by designating at least one representative to participate as a core or advisory member.
  • Core Members: Individuals who attend quarterly coalition meetings and are actively engaged in the development, implementation and evaluation of the coalition’s work plan, including involvement with at least one action team.
  • Advisory Members: Individuals who are not actively engaged in the development, implementation or evaluation of the coalition’s work plan, but may attend quarterly coalition meetings and receive and respond to coalition communications.
  • Funders: Organizations providing financial and/or in-kind support to the coalition.
